Road surface coming away, potholes forming

Road surface is falling apart quickly and potholes forming causing cars to swerve as they come round corner and into path of oncoming cars.

Exposed metal drain cover, dangerous for cyclists and motorcycles. Also, when your workforce poorly repair 1 pot hole out of many repairs that need carrying out on a corner that is becoming more dangerous with traffic avoiding holes/damage and nearly having head on collisions, why do they deliberately destroy decent and well kept grass verges? There is plenty of room for them to park on the road without them destroying the road side. The damage to this corner is going to cause an accident in the near future unless it is attended to ASAP. Further more, a lorry or something similar has driven over and broken the access cover to the water hydrant in the grass verge on the same corner. This again needs urgent attention, in case it is required in an emergency.
