Deep Pothole burst my tyre.

Several potholes which are impossible to avoid when traffic is coming in the opposite direction. I was travelling at 20mph at about 8.30am with a lot of school traffic about on 28/2/20 when my tyre was punctured after being forced to drive straight in to this pothole.

    The hole on the left is about 5 inches deep from the level of the road surface. Difficult to take a photo of it whilst holding the tape measure but the edges on it are obviously capable of ripping holes in the rubber of car tyres and doing expensive damage to Alloys. Can I have a claims form emailed to the address I've provided please? Thank you.
