Newly repaired road surface failing - significant debris in road

This pot hole almost lead to a head on crash today - my wife was driving towards northampton and a car coming the other way (towards daventry) swerved into her lane to avoid this pothole that was repaired badly recently and has begun to fail again. She had to put her breakers on hard to avoid an accident. This poor initial repair, which did not consider the poor condition of the road adjacent to the repair, put my family at risk - there were also two children in the car. There are large sections of this road surface that fail on a regular basis - the drainage is poor (see photo which sown it recently, including a chunk of the failed new repair) - cars regularly move into the other lane to avoid the multiple pothole. Please can you consider a better repair of this whole section rather than ad hoc repairs. Happy to discuss - indeed I would appreciate a substantive response and not just a notification it’s been assessed and will be repaired.
