Deep undulation in footpath, severe trip hazard

Sent to Northamptonshire Highways less than a minute later. ref: 3618108.

This pavement has been uneven prior to us moving into the premises (27 Market Place, Brackley) 18 months ago. I have witnessed several people lose their footing as a result of this. Today, a lady of 102 years old lost her balance due to this issue. She fell heavily onto her hip and arm and an ambulance called. She initially thought she had broken her hip, however it later transpired that this wasn't the case. It is only a matter of time until somebody seriously injures themselves as a result of this. The lady in question was trying to right herself and staggered across the pavement and was inches away from hitting her head on a parked car.
