Loose drain cover causing a very loud noise when driven over

Sent to Northamptonshire Highways less than a minute later. ref: 4452212.

The drain cover has not been fitted correctly after it was mended 3 months ago and has been making a horrendous noise when driven over ever since. This is causing lack of sleep and affecting my mental health. Please can someone look at this urgently.
